I consent to have my child(ren) participate in the programs offered by RISE PAC. I understand that participating in dance is participating in a physical activity and that risks are involved, as with any dance class, sport, or physical activity. I voluntarily assume the risks associated with such participation and do not hold Rise Performing Arts Center responsible for injury, property loss or damage, while my child is participating at Rise Performing Arts Center, located at 1140-1142 Riverside Drive, Los Altos, CA. I, my executors, or other representatives hold RISE PAC harmless, and waive and release all rights and claims for damages that I or my child may have against RISE PAC, its staff or its representatives.
